YOLT- When Bond didn't seem to care about Aki's death.
SF- Severine's death.
We've been over this topic many times, but I got the impression Bond was deeply affected by Aki's death (it was the movie itself that was indifferent; YOLT was a light adventure romp mainly, not a doomed romance story) and Bond seemed more uncaring about Severine. And Bond is a seasoned killer who learned to bottle up his feelings about people dying horribly (and Bond coldly proclaimd the "the bitch is dead!" after Vesper died and at the same time you know he'll never get over her death either).

I also feel Bond was upset over Aki's death. He didn't have the time to grieve properly as the Americans had changed the launch date. He had to put personal feelings aside for the sake of the mission. I think after the mission was completed, Bond grieved properly for her off screen.

People and the most of the movies in general tend to dwell on the racy chases and fun locations, not on the amount of heavy emotional suffering and heartbreak that Bond goes through a lot.
And speaking about another incident that induces a lot of anger and heartbreak is Bond's intimidation of Ms. Anders and his failure to protect her from Scaramanga.

There in lies my problem with TMWTGG. I quite enjoy many aspects of the film, but if it could decide what kind of movie it wanted to be, it would have worked much better for me. There are way too many dramatic shifts from very silly to very dramatic and sad.
I find Andrea Anders to be one of the most tragic characters in Bond. A mistress essentially held captive and her one hope and the hero of the film doesn't trust her and subjects her to similar abuse. Finally, when she is starting to gain Bond's trust, she is killed. Definitely a sad moment. Wouldn't say it pisses me off though. The issue is the whole Mary Goodnight & Sheriff Pepper sequence that immediately follows definitely lessens the emotional importance of the death in the film.

DAF - Having a black woman "transform" into an ape in a scene at the Vegas hotel, Circus Circus. Very racist.
Stultifying in its racism . . . and considering it was only a few years after the death of Martin Luther King, Jr., rather baffling that no one was sensitive to showing it on film . . .
Several scenes left me unhappy, as they were intended to. The deaths of Quarrel, Kerim, and Aki, for instance. These were meaningful characters that I liked, but at least their demises worked within the story to make us feel sorrow while at the same time wishing Bond's revenge. But other demises have been handled poorly, and these bothered me for that reason most of all. Examples would be Sharky and Severigne, whose dispatching was handled in a particularly clumsy way by the writer and director. It's obvious the people making the films thought the characters were more throwaway than they really were, and the funny thing about Severigne is that she made a better Bond girl than the one in Quantum of Solace, even though they both had the stereotypical violent and tragic past.
But overall, the biggest disappointment came from Goldeneye, which should have been a better movie. I waited six years and was terribly disappointed.
