Why was each title/story chosen for the next film?

Up until LTK, and of course CR and QoS, each film was either based on a short story/novel or at least used a title, which was obviously picked before the film before had even been released. But why were the novels/stories chosen when they were? Obviously there are a few well known ones, such as FRWL chosen due to JFK's inclusion of the novel in his favourite novels list, and MR chosen to capitalise on Star Wars, but why were other stories picked? Was it just a random decision? And why was QoS chosen as the title for a direct sequel to CR? It'd be really interesting to hear if anyone knows why each title was chosen when it was.
Also, why was FYEO chosen before MR? Obviously that quickly changed, but why was the FYEO title going to be used before all the available novels had been used?

Moonraker had much more cinematic potential than some of the earlier adaptations, especially the two most recent titles, which were very dull books, same with Diamonds Are Forever
