Does anyone know how many times "Bond, James Bond" and "Shaken, not stirred" are said in all 20 of the Bond Movies?

The reverse in YOLT of 'Shaken not stirred' is because it is Japanease custom to agree with the host! You will find in quite a few scenes Bond agree's with Tiger as a mark of respect.

Yes, it's been debated previously whether this was a mistake or not. Did Charles Gray (as Henderson) intend to say "Shaken, not stirred"? Connery's reaction may well suggest this. Did Lewis Gilbert and the producers then realise Gray's mistake, or not?

I am of the belief that it was all in the script. Henderson is mean't to say "Stirred, not shaken"? Bond realises the mistake but decides to remain the gentleman to his host.

If you listen to the director's commentary on the DVD, Gilbert admits it was a mistake that he missed and they did not go back to correct it. On other boards, people dismiss this explanation, saying that Gilbert was too old to remember things correctly. So it's going to boil down to this: believe whatever you want to believe.
